Comments and Suggestions

We always welcome feedback about our services. If you have a comment or suggestion about the practice, please let us know. Any member of staff will be happy to listen to you. Or, if you prefer, you can send us a letter.

Do you have a Complaint?

We hope most problems can be sorted out quickly and easily by the reception staff. If your problem cannot be sorted out, you can make a written complaint.

How to make a written complaint.

Let us know as soon as possible. We cannot investigate incidents that happened more than 6 months ago.

Address your letter to either Dr Fields, Dr Livingstone or to our Practice Manager.

Describe in detail:

  1. what happened
  2. why you are concerned
  3. and what you would like to happen in future.

What we will do.

  • We shall acknowledge your complaint within two working days, either by letter or by telephone.

  • We shall look into your complaint, and reply to you within 10 working days.

  • We will try to find out what went wrong, and how we can put it right.

If we have made a mistake, we will apologise and try to make sure it doesn't happen again. If there has been a misunderstanding, we shall explain what happened and why.

Complaining on behalf of someone else.

For reasons of medical confidentiality, we would normally expect to deal directly with the patient concerned. If you wish to complain on behalf of someone else, we have to make sure they give their permission for this.

If you are still not satisfied:

You can write to Lincolnshire Primary Care Trust, Cross O'Cliff, Bracebridge Heath, Lincoln, LN4 2HN.
